EverWeb 4.6 and later add rating fields to the Contact Form Advanced widget: Star Rating, Emoji Scale and a 0 to 10 Rating Scale. Together with a comments box, they make a short feedback form or survey that looks like the rest of your website, without a third-party survey tool.
Who uses this
- A restaurant prints a QR code on its bills linking to a two-question feedback page.
- A bed and breakfast emails guests after their stay and asks for a star rating and a comment it can quote.
- A trainer asks everyone to rate the course and suggest improvements.
- A service business asks the classic "How likely are you to recommend us?" question on a 0 to 10 scale.
What you need
- EverWeb 4.6 or later for the rating fields. They work without the add-on: answers arrive by email.
- The Contact Forms Enhanced add-on if you want every answer stored online, sorted and exported to a spreadsheet, or a thank-you email to the visitor. See What is the Contact Forms Enhanced Add-on?
The form basics are in Build a custom form: quote requests, registrations and file uploads.

Set it up
- Make a page for the form, such as Feedback, and drag Contact Form Advanced onto it. Open Inspector > Widget Settings.
- If you have the add-on, choose it in Contact Form Enhanced Add-on and type a Contact Form Name, such as Guest feedback.
- Add the fields from the plan and choose each Control Type. Keep it short: three to five questions get far more answers than twenty.
- Use a Hidden Field to tag where the answer came from. Visitors don't see it. If you have feedback forms on several pages, or one page for a QR code and one for an email link, give each copy a different Default Value, such as Table QR code or After-stay email.
- Pick a Form Style that suits your website, such as Soft or Card.
- Set Submit Caption to Send feedback, choose a thank-you page in Forward To Page, and publish with File > Publish Entire Site.
To ask for feedback without a separate page, put the form in a Popup Window and tick Close popup window after sending: see How do I open a popup window or enlarge a photo when someone clicks?
What you receive
- An email for each answer, with every rating as a number.
- With the add-on, every answer in your Client Area under Contact Form Submissions > View Form Submissions. Sort by any column, search, or Export to a CSV file. Open it in Excel or Numbers to average your star ratings or count how many people scored you 9 or 10.
Good to know
- Answers aren't shown on your website automatically. The form collects feedback privately. To show reviews, copy the ones you like (with permission) onto a Testimonials page yourself.
- Emoji Scale sends a number, not the face. With five faces, 1 is the first face and 5 the last.
- No follow-up questions based on the score. Fields can't appear only for low ratings. Ask one optional question for everyone, such as "Anything we could do better?"
- A Consent field starts as required, so use a Toggle for optional yes/no questions such as "you may quote me".
- Public forms attract spam. Turn on Use reCaptcha SPAM Protection and choose a Captcha Provider.
- To see how many people visit the feedback page, see See how many people visit your website: Website Stats and Google Analytics.
