Run an online quiz, test or application form with a multi-step form

A long form with Step Break pages, in EverWeb 4.6 and later, works well for quizzes, tests and applications. Each page has a title and a Next button, so people answer one section at a time. The form collects the answers and sends them to you. It doesn't mark them: you do that yourself.

Who uses this

  • A driving school gives pupils a practice theory quiz and goes through the answers at the next lesson.
  • A training company asks course members ten questions at the end of each module.
  • A small business takes job applications, with a CV upload and a few questions.
  • A foundation or school takes scholarship or entry applications, with an essay upload and an application fee.

What you need

  • EverWeb 4.6 or later for Step Break pages.
  • The Contact Forms Enhanced add-on if you want File Upload (for CVs and essays), a fee, a confirmation email, or every answer stored online and exported to a spreadsheet. See What is the Contact Forms Enhanced Add-on? A simple quiz by email works without it.

For a quiz, use a Radio Button for each multiple-choice question and a Text Area for written answers. Put a Step Break every five or so questions.

Set it up

  1. Drag Contact Form Advanced onto a page and open Inspector > Widget Settings. If you have the add-on, choose it in Contact Form Enhanced Add-on and type a Contact Form Name, such as Job applications 2027.
  2. Add your fields in order. Start with the questions that apply to everyone.
  3. Add a Step Break where each new page should start. Its text is the title of the next page. Don't start the form with a Step Break, and don't put one inside Columns.
  4. Tick Required field on the questions that must be answered. Next only moves on when that page's required fields are filled in. A required Drop Down Menu needs a prompt such as "Choose a position" as its first option.
  5. For uploads, set Accept on each File Upload field. Each field takes one file.
  6. While designing, Preview first form page only shows page 1. Untick it to check the others.
  7. Optional fee: in the Payments section tick Accept Payments, add your Stripe and/or PayPal details, and add a Consent field such as "I agree to pay the application fee" with Price When Checked 25.
  8. Choose a thank-you page in Forward To Page, publish with File > Publish Entire Site, and fill in the whole form yourself on a phone.

What you receive

  • An email with every answer, labelled with your question, each time someone sends the form.
  • With the add-on, every entry in your Client Area under Contact Form Submissions > View Form Submissions, with CVs and other files in the Attachments column. Export gives a CSV file with one row per person and one column per question, the quickest way to mark a quiz or compare applicants in Excel or Numbers.
  • Send Confirmation Email (add-on, needs spam protection turned on) can tell applicants you've received their application.

Be clear with people about what it does

  • No automatic marking or scores. The form can't check answers, add up a score, or show people their result. Tell them when and how you'll send results.
  • No timer. You can't limit how long someone takes, or stop them looking things up.
  • No answer-based questions. Fields can't appear or hide depending on an earlier answer. Write "If you answered Yes, tell us more" and keep that field optional.
  • People can send the form more than once. Ask for an email address so you can spot repeats.

Good to know

  • For job and scholarship applications, check the privacy rules that apply to you, and say in your privacy notice how long you keep applications. Delete old ones from the Client Area.
  • Don't ask for card numbers, bank details, passwords or government ID numbers in the form. Ask for anything like that later, through your normal process.
  • The fee is a one-off payment, and you get the application marked Paid only once the payment is confirmed. If someone doesn't finish paying, it reaches you a few hours later marked Unpaid order.
  • Long forms are easier on phones with one question per line. Columns stay side by side on phones, so only use them for two short fields.
