Photographers: model releases, contracts and client questionnaires online

With EverWeb 4.6 or later and the Contact Forms Enhanced add-on, a Contact Form Advanced form can collect a signed booking contract or model release with the Signature field. Any form can carry a client questionnaire, such as a wedding day timeline or the details of a newborn shoot. You get every answer by email and, with the add-on, in your Client Area. EverWeb provides the form: the wording of your contract and release stays yours.

  • A wedding photographer has couples sign the contract when they book, then sends a questionnaire six weeks before with the timeline and the family photo list.
  • A portrait photographer has models sign a release on an iPad at the studio.
  • A pet photographer asks for the pet's name, breed and temperament before the shoot.
  • A commercial photographer has a client approve usage terms before a product shoot.

A note on signatures

Rules for electronic signatures differ by country and by type of document. Check with your lawyer or professional body that a signature on a web form is enough for your contracts and releases. Use the wording they recommend.

A contract or model release form

  1. Put the full contract or release on its own page (untick Include page in navigation menu), or as a PDF.
  2. Drag Contact Form Advanced onto a page, choose your add-on in Contact Form Enhanced Add-on and type a Contact Form Name such as Wedding agreement.
  3. Add the fields below, then publish with File > Publish Entire Site.

For a model release, add:

Parent or guardian's full name (if under 18)TextBox

Forms can't show or hide fields depending on an answer, so say "if under 18" in the label.

Signing at the studio: open the published form on a tablet. The model signs with a finger, and the Clear button lets them try again. Set Forward To Page to a page that says Thank you, please hand the tablet back, with a button back to the form for the next person.

A client questionnaire

Use Step Break to split a long questionnaire into short pages with Back and Next buttons. Each Step Break's text is the title of the page after it.

For other shoots, change the questions: a pet photographer might use a Radio Button for temperament (Calm, Excitable, Nervous with strangers), and a real estate photographer might ask how to get into the property.

Turn on Send Confirmation Email (it needs the add-on and spam protection) and put %allformdata% in its content, so the client gets a copy of their answers.

What you receive

An email with every answer. The signature comes as a picture (PNG) attached to the email, so set Mail Sending to SMTP Details for this form. With the add-on, the submission and any uploaded photos are also in your Client Area under View Form Submissions, where you can download attachments and export to CSV.

Good to know

  • Ask only what you need. Don't ask for card numbers, bank details or ID numbers. Check the rules on keeping clients' personal details that apply to you, and delete old submissions you no longer need.
  • Get the padlock first, so the form is sent securely: see My site says 'Not Secure': how to get HTTPS (SSL) on your website.
  • Columns keeps fields side by side on phones too, so use it only for two short fields. Preview first form page only shows page one while you design.
  • Forms send only from your published website, not Preview.
